ABSTRACT

PURPOSE: This study examined the clinical success rate of Mg titanate implants (M Implant system, Shinhung, Korea), which employ a Mg coating method, by evaluating the marginal bone loss and implant stability using radiographs and Osstell(R), over a 1 year. MATERIALS AND METHODS: The locations of the implants placement were divided into 4 areas; the maxillary and mandibular premolars and molars. In the maxilla, 8 and 9 implants were inserted in the premolar and molar areas, respectively. In the mandible, 11 and 51 implants were inserted in the premolar and molar areas. Marginal bone loss and ISQ of all implants (79) were measured after insertion, mounting the prosthetic appliance, and 1, 3, 6, and 12 months after loading. The marginal bone loss was measured from the radiograph using XCP bite, which was customized, and the implant stability measured using Osstell(R). Fisher's exact test (alpha=.05) was used to compare the success rates of each region. RESULTS: The mean marginal bone loss for the upper and lower jaws were 1.537 mm and 1.172 mm. The mobility showed a non-significant reduction or increase according with time. The success rates were accounted for 94.12% and 98.39% in the upper and lower jaws; the premolars and molars were accounted for 100% and 96.67%. The two cases of early failure resulted from failure of primary stability during implant insertion. The late failures were not observed for 1 year after adding a loading to the implants. CONCLUSION: The Mg titanate implant showed good primary stability and good clinical results in both healing and function.

ABSTRACT

Dr Lillehei (1918~1999) pioneered cardiac surgery with his landmark operations using cross-circulation in 1954 and 1955. With his dedications to open heart surgery, he is generally considered to be the father of open heart surgery by many medical historians. Dr Lillehei expanded his contributions to cardiac surgery with training 134 cardiothoracic surgeons at the University of Minnesota Hospital and he trained an additional 20 surgeons at the Cornell Medical Center. Dr Lillehei's trainees came from all over the world and Dr YK Lee (1921~1994) of Seoul National University was among them. He joined the University of Minnesota Hospital in 1957 as a part of the Minnesota project. During his stay for two years, in addition to experimental research, he learned clinical cardiac surgery as part of Dr Lillehei's team. In 1959, after returning to Korea, Dr Lee began his career as a full-time cardiac surgeon with establishing the Division of Cardiac Surgery at Seoul National University Hospital. Yet he encountered many difficult barriers in the process. During that time, Dr Lillehei was willing to share his experience and he provided many valuable resources for cardiac operations. With Dr Lillehei's kind help, the open heart surgery program was gradually and successfully established at Seoul National University Hospital. These two surgical titans from across the Pacific Ocean died in 1994 (Dr Lee) and 1999 (Dr Lillehei). They are gone, yet the proud Korean people have not forgotten them.

ABSTRACT

STATEMENT OF PROBLEM: Bioactive materials must have the ability to spontaneously form a bone like apatite layer on their surface and induce direct biochemical bonding to bone. A simple chemical treatment via alkali and heat has been revealed to induce bioactivity in titanium. PURPOSE: The purpose of this study was to evaluate the surface characteristics and stability of alkali and heat treated implants. MATERIAL AND METHODS: Specimens were divided into three groups; group 1 was the control group with machined surface implants, groups 2 and 3 were treated with alkali solutions and heat treated in the atmosphere and vacuum conditions respectively. The surface characteristics were observed with FESEM, XPS, TF-XRD and AFM. Stability was evaluated with the resonance frequency analysis, periotest and removal torque values. One-way ANOVA and Duncan test were used for statistical analysis. RESULTS: 1. Groups treated with alkali and heat showed similar characteristics. Groups 2 and 3 showed high compositions of Na ions on the surface with sub-micron sized pores compared to group 1. Group 2 showed mixed compositions of anatase and rutile with superior contents of rutile. 2. Resonance frequency analysis : The ISQ of group 2 showed significantly higher values than that of groups 1 and 3 at 12 weeks. The ISQ of groups 1 and 2 showed significant increase after 4 weeks, and the ISQ of group 3 increased significantly after 2 and 4 weeks respectively (P < .05). 3. Periotest: The PTV of groups 1 and 2 showed significant decrease after 4 weeks, and the PTV of group 3 showed significant decrease after 2 and 4 weeks respectively (P < .05). 4. Removal torque analysis: The removal torque value of group 2 was significantly higher than those of groups 1 and 3 at 2, 4 and 8 weeks. The removal torque values of groups 1 and 3 showed increase at 4 and 12 weeks, but the removal torque value of group 2 showed increase after 4 weeks (P < .05). CONCLUSION: An oxide layer with appropriate crystal structure and amorphous sodium titanate layer can be obtained on titanium implants through alkali and heat treatment in the atmosphere, and even alkali and heat treatment in vacuum conditions, provided a bioactive surface containing sodium. These surface layers can be considered to be effective for enhancement of osseointegration and reduction of healing period for implant treatment.

ABSTRACT

The nature of the implant surface can directly influence cellular response, ultimately affecting the rate and quality of new bone tissue formation. The aim of this in vitro study was to investigate if human osteoblast-like cells, Saos-2, would respond differently when plated on disks of magnesium titanate and machined titanium. Magnesium titanate disks were prepared using Micro Arc Oxidation(MAO) methods. Control samples were machined commercially pure titanium disks. The cell adhesion, proliferation and differentiation were evaluated by measuring cell number, and alkaline phosphatase(ALPase) activity at 1 day and 6 day after plating on the titanium disks. Measurement of cell number and ALPase activity in Saos-2 cells at 1 day did not demonstrate any difference between machined titanium and magnesium titanate. When compared to machined titanium disks, the number of cells was reduced on the magnesium titanate disks at 6 day, while ALPase activity was more pronounced on the magnesium titanate. Enhanced differentiation of cells grown on magnesium titanate samples was indicated by decreased cell proliferation and increased ALPase activity.

ABSTRACT

BACKGROUND: CSF can be leaked from the nose or ear due to fractures, tumors or surgical procedures in the skull base region, and the threat of impending meningitis necessitates early identification of it. Since 2-transferrin occurs practically in cerebrospinal fluid (CSF) and not in other body fluid, its detection from the rhinorrhea or otorrhea can be used for the diagnosis of CSF leakage. We carried out immunofixation-silver stain (IF-SS) method for detection of 2-transferrin in the CSF in order to know optimal identification condition of specific cerebrogenic marker. METHODS: The fresh CSF sample was collected by spinal tapping. 2-Transferrin was estimated by quantifying the total transferrin by nephelomertry (Behring, Germany). 2-Transferrin of CSF was identified by electrophoresis using Titan gel high resolution protein system (Beckman, USA), immunofixation with anti-human transferrin antibody (Dako, Denmark) and then stained with silver nitrate. Serial dilutions of CSF were performed to know the detection limit of 2-transferrin. To know the influence of blood mixing, tests for mixed specimen of serum and hemolysate in CSF were performed. To evaluate the specimen storage condition, tests for different temperature and storage time were performed . RESULTS: By IF-SS method, identification limit of 2-transferrin was 0.5 mg/dL in 1:4 diluted CSF with distilled water. And 2-transferrin could be detected in condition of mixing serum protein (7.5 g/dL) or hemoglobin (13 g/dL) with CSF up to 6 : 4. At various sample storage condition, such as 37degrees C, room temperature, and 4degrees C, band intensity decreased abruptly after 1 day, and it was not detected 5 days later. Mean while, in -20degrees C and -70degrees C, 2-transferin band was detected after 10 days. CONCLUSIONS: IF-SS method was sufficiently sensitive and specific for invalidation by blood contamination, and seems to be used as effective identification of 2-transferrin in the CSF without sample concentration, less diagnostic test for CSF leakage.

ABSTRACT

There was no definitive reported cases of urinary calculi. This study is to estate the value of urinary calcium, magnesium, uric acid and magnesium/calcium ratio in cases of urinary calculi. The amount of calcium, uric acid and magnesium in 24 hour urinary excretion was measured by OCPC, Uricase and titan yellow method in 56 cases of upper urinary calculi compared with 35 control group.The results were summerized as follows.l. The mean values of 24 hour urinary excretion of calcium were 193+/-26mg/day in stone formers and 15l+/-25mg/day in control group. The 24 hour urinary excretion of calcium in the stone formers were significantly higher than those of the control group. 2. The mean values of 24 hour urinary excretion of uric acid were 498+/-40mg/day in the stone formers and 371+/-6mg/day in the control group. The 24 hour urinary excretion of uric acid in the stone formers were significantly higher than those of the control group. 3. Urinary magnesium values have not been shown any significant differences between the group with urinary calculi and those of control group. 4. The mean values of 24 hour urine of magnesium to calcium ratio was 1.01+/-0.95 in stone formers and l.42+/-1.31 in the control group. The magnesium to calcium ratio in the stone formers were significantly lower than those of the control group. These results suggest that increased urinary calcium and uric acid level may play some role in the genesis of urinary stone but urinary magnesium value was not different in the two groups. We think that a decrease in the urinary magnesium to calcium ratio was the results of increased excretion of calcium rather than lowered excretion of the magnesium for the stone formers.

ABSTRACT

The authors studied the diagnostic value of the oligoclonal band in cerebrospinal fluid in various neurological disorders. Cerebrospinal fluid of thirty-one patients including eight cases of multiple sclerosis was tested for oligoclonal band by agarose gel electrophoresis using the Helena Titan Gel High Resolution Protein System. Oligoclonal bands were detected in eleven cases of various disorders (two cases of optic neuritis, three cases of multiple sclerosis, three cases of CNS cysticercosis, and three cases of other disorders). This study suggested that detection of oligoclonal band in CSF was of usdful diagnostic aid in multiple sclerosis in addition to CSF IgG/Albumin ratio and CSF IgG index.
